- Pay by Paypal
If you are under 18 you must have a parent or guardian with you and they must fill in this form.
In a public space it may not be possible to maintain complete confidentiality during discussions. Your privacy and dignity will be respected and the therapist can work through clothing and towels as available. If you have any questions or information you prefer to keep private write it on the disclaimer.
